Based on the topic and outline from previous units, you will make a formal oral presentation to your instructor and peers. You will be required to present for 10–12 minutes using the Adobe Connect technology within the course area. Assume your role as a human service or public service leader for all assignments in this course. In this assignment, focus on gathering information about continuum of care services and writing a review of those services. Use the list below to select two continuum of care services provided for a diverse aging-adult population in your community:

Part 2: Critical Review

Part 3: Incorporation Into Action Plan

For this assignment, identify a problem or issue that you learned about while researching specific organizations that could be the focus of a recommendation report and plan of action. Consider the information you gathered about the facilities (for example, rights and responsibilities and caregiver resource coordination) to identify a potential issue. Explain how you would incorporate the issue you identified into a recommendation report and plan of action.
