Moreno Medical Center – Issues in Managed Care

CASE STUDY

Moreno Medical Center (MMC) is a 600-bed, full-service acute care facility located in the Northwest Kelsey community. The hospital is a physician-owned, for-profit hospital that takes pride in the provider-patient relationship. With recent changes in managed care, every dollar, process, and interest of the patient serves as a common ground for making an organization more efficient.

 

To remain competitive in the marketplace, the management team at MMC decided to take a look at various processes within the hospital and noted that the overall patient experience in the outpatient surgical clinic was less than satisfactory. The main problem seems to focus on long delays throughout the entire admission to discharge process. If the hospital wants to maintain its competitive position, the administration will have to redesign the current process to improve patient satisfaction.

 

The hospital administration appointed an internal quality consultant to facilitate the planning process. The quality consultant for this new organizational role is an experienced intensive care RN with a business degree. She understands that health care is a personal encounter and that patients and families expect to have their priority needs met when experiencing a health care issue.

 

The quality consultant decided to work closely with the surgical department RN administrator who would be a powerful change agent in leading the improvement process. During the review of the surgical department processes, the administrator and consultant identified several major issues:

 

·       Surgeons delaying surgery times causing backlog of surgery schedule

·       Other hospital departments (e.g., admitting, finance, lab) conducting separate encounters with surgical patients without regard to the surgery schedule

·       Long wait times for patient transportation from surgical area to recovery room

·       Ancillary staff providing patient discharge instructions when nurses are busy

·       Nursing staff displaying a culture that the procedure, not the patient, is the priority

 

The surgical department RN administrator acknowledges that changes need to occur and the new process must be guided by what is in the best interest of the patient. He developed a new vision statement for the department with its first priority to provide a patient and family focused quality care culture.

Analyzing the Losses of Others

 

Background:  This project will involve interviewing three individuals about their most significant loss in Life.  You will then analyze your findings in relation to concepts from the text/class.

 

 

 

  1. Interview 3 people about the thoughts and feelings they experienced relative to the GREATEST loss in their lives.  Ask each person, “What is the greatest loss that you have experienced in your life so far?”  Remember the loss does not have to be about death!  Let each person decide his/her own greatest loss.  If at all possible you will want to interview relatives for this project.  You may be surprised at the insights you gain into your own family.
  2. Before you interview your interviewees, be sure to compose a list of questions that you will ask.  Ask each person the same questions.  You may also use some of the sample questions listed below.  You must ask a minimum of five (5) questions.

     

 

  • What was your greatest loss so far in your life?
  • What exactly happened?

“What Are the Major Ethical Issues in Conducting Research?”

Ethical Dimensions of Research Studies

In the best-selling book, The Immortal Life of Henrietta Lacks (Skloot, 2010), the author highlights the true story of an African-American woman who died in 1951 from cervical cancer. What makes her story unique is that prior to her death, cells from her tumor were removed and successfully grown in a petri dish. This was the first time scientists were able to successfully replicate cells outside the body, and it is estimated that billions of Lacks’ cells have been used in medical research. However, Henrietta Lacks was never asked for permission to take a sample and her family was never made aware of the widespread use of her cells. Although the culturing of her cells has been pivotal for advancing research, strong ethical concerns later arose about using these cells without patient or family approval.

This week’s readings describe historical examples of unethical research, such as a study of syphilis among African-American men in which treatment was withheld and a study in which live cancer cells were injected into elderly patients. Critiquing a Change Effort

As a nurse leader, you need to have the skills and knowledge to collaborate and communicate with those who plan for and manage change. This capacity is valuable in any health care setting and for many different types of change. Furthermore, it is essential to be able to evaluate a change effort and determine if it is promoting improved outcomes and making a positive difference within the department or unit, or for the organization as a whole.

To prepare:

  • Review Chapters 7 and 8 in the course text. Focus on the strategies for planning and implementing change in an organization, as well as the roles of nurses, managers, and other health care professionals throughout this process.
  • Reflect on a specific change that has recently occurred in your organization or one in which you have worked previously. What was the catalyst or purpose of the change?
  • How did the change affect your job and responsibilities?
  • Consider the results of the change and whether or not the intended outcomes have been achieved.
